Hand surgery:Plantar fibromas (childrens digital fibromas) — Surgery resection. Treatment abroad
Digital arm fibromatosis is single or multiple benign tumors developing in children during the first years of life. The formations are commonly located on the lateral surfaces of the fingers, and sometimes on their back side. They look like pink nodules from several millimeters to 2 cm large. The tumor cells are derived from the connective tissue elements of the upper layer of the dermis. Pathology treatment depends on the severity of its course.
When fibromatosis is suspected, the biopsy followed by histological examination is mandatory. It is important to differentiate the formations from exostosis, extra toes, verrucous units and other similar pathologies.
If the diagnosis is confirmed, the patient is subject to a dynamic follow-up. It happens that fibromas disappear spontaneously over 2 to 3 years. In some cases, the expert may prescribe glucocorticosteroids or fluorouracil at his/her sole discretion.
When the tumor nodules are enlarged and the functional disorders are developed, the surgical intervention is indicated. Under general anesthesia, the surgeon makes an incision on the skin and excises as much fibroma as possible. The edges of the wound are sutured with bioresorbable sutures.
In 60% patients, the digital fibromatosis recurs after surgery. In such cases, the issue of a broad surgical excision is considered. In order to eliminate the eventual remaining tumor cells, the radiotherapy can be prescribed. In modern clinics, the irradiation is carried out using advanced techniques, which cause virtually no damage to healthy tissues. The following techniques are practiced:
- Proton beam therapy. The pathological focus is radiated by proton beams from different sides. The maximum energy is focused on a clearly limited area, without distribution to the adjacent areas.
- Conformal therapy. Tumor residues are also affected from different angles with low doses of gamma radiation. Irradiation technique is developed using special software.
- Intensity-modulated radiotherapy. Radiation is dosed in view of three-dimensional configuration of pathological areas.
The procedures are conducted daily on an outpatient basis for a few weeks.Hide
