Coxarthrosis – Diagnostics

The main steps in the establishment of a diagnosis are:

  • Clinical diagnostics. The doctor assesses patient's complaints. In the case of coxarthrosis, pain is usually observed not in the hip joint, but in the knee, groin, or buttock. The pain increases after the physical exertion. During the examination, the doctor detects the atrophy of the gluteus muscle on the lesion side. There is a soreness while the palpation of the inguinal region. One leg may be shorter than the other one.
  • Laboratory diagnostics. It is impossible to diagnose coxarthrosis with the help of the laboratory methods. But they are required for differential diagnostics in order to exclude other diseases that cause damage to the hip joint. In the case of gout, the concentration of the uric acid in the blood increases, and in the case of arthritis, the inflammation markers appear. In the case of coxarthrosis, such changes are not observed.
  • Radiography is an image of the pelvis and both hip joints. This is the main method of coxarthrosis diagnosing. Classical signs of dystrophic changes in the joint are determined exactly on the X-ray. Such signs include the narrowing of the joint gap, the growth of the osteophytes along the edges, and subchondral sclerosis. Often, X-ray changes do not correlate with the clinical picture. Therefore, medical treatment of coxarthrosis can be prescribed even when the significant signs of disease are absent on the X-ray.
  • MRI is performed as a clarifying method. It allows to receive a 3D image of the hip joint structure. Also, it can be used for surgery planning.

Coxarthrosis – Treatment

In the case of coxarthrosis, some treatment is performed to reduce the pain, to maintain or restore normal working capacity. It is important to prevent the further destruction of cartilage and disease progression.

Conservative treatment methods in the case of coxarthrosis include:

  • Physiotherapy. First of all, training is required in order to strengthen the quadriceps femoris muscle.
  • Physiotherapy. Cold, heat, electrostimulation of nerves, etc.
  • Drug therapy.

The purpose of drugs administration is to reduce the pain syndrome. Non-steroidal anti-inflammatory drugs are prescribed. If they are ineffective, narcotic analgesics can be used. When the signs of inflammation are present (in combination with the arthritis), glucocorticoids are prescribed. Hyaluronic acid can be injected in the joint every few months.

At the certain stage of the development of the disease, any treatment may become ineffective. In this case, the endoprosthesis of the hip joint is performed. The main indications for the operation are the following: chronic pain syndrome, which is poorly blocked by analgesics and significantly affects the quality of life, or a significant violation of joint function with a sharp restriction of the movements volume.
