Tinnitus is a subjective noise in the ears. No one, except the patient, can hear this noise. The doctor can’t detect sound fluctuations with the help of special examination techniques. Thus, the appearance of extraneous sounds is not associated with acoustic stimuli.
Up to 5% of world's population suffers from chronic tinnitus. Often this noise becomes dominant. The patient concentrates his attention on it, and this causes stress, depression, sleep disturbances, irritability, inexplicable fears.

Diagnostics of Tinnitus
Diagnostics begins with a clinical history taking. The doctor finds out the following aspects:
- Type of noise
- Intensity
- Time of occurrence
- The patient's attitude to his feelings
One of the four stages of tinnitus can be established. At the first stage, this condition is compensated (the patient gets used to the noise). At the second one, the patient concentrates on it. At the third stage, there is irritability or depression. At the fourth one, the patient loses his working capacity.
Further examinations are aimed at identifying the cause of tinnitus. The doctor must also distinguish it from the objective noise. This is a condition, when the sounds which the patient hears are actually present. They may have a vascular or muscular origin.
In the case of noise or ringing in the ears, German doctors perform such examinations as:
- Dix-Hallpike test
- Speech audiometry
- Timpanometry
- Brainstem evoked response audiometry
- Otoacoustic emission test
- Head impulse test
- Computed tomography of the petrous pyramid of the temporal bone
- MRI of the labyrinth with contrast enhancement
If the doctor establishes the exact cause of tinnitus, the illness can be cured completely. In this case, the noise disappears. However, the main etiological factor can be determined only in 5% of the cases. In other cases, the origin of the symptom remains unclear.

Treatment of Tinnitus
In Germany, doctors have great experience in the treatment of tinnitus. According to the German Tinnitus League (Deutschen Tinnitus-Liga), about 3 million of people suffer from this disease. In addition, 250,000 of new cases are registered annually.
In the case of tinnitus complex treatment is performed, such as:
- Drug therapy
- Physiotherapy, including fluctuating currents
- Psychotherapy
Drug therapy includes vasoactive drugs, antidepressants, sedatives or hypnotics.
In German clinics, the TRT (tinnitus retraining therapy) method is used. The purpose of this method consists in training the patient which lasts for several days. The doctor convinces the patient that the noise doesn’t affect his health or threaten his life. The sound therapy is carried out. The patient listens to "the white noise". It is perceived positively by the person and consequently, after a certain time period, the brain doesn’t notice it at all. An example of such a noise, is the sound of falling rain drops or the sound of the sea. "The white noise" is selected individually. These measures allow not to eliminate tinnitus but significantly reduce its severity, as well as a negative influence on the person's quality of life.
The hearing is reduced in 85% of people who suffer from tinnitus. In case of deafness, a hearing aid is selected for the patient. Often, the amplification of external sounds suppresses the internal ones.
After the course of treatment in the clinic, the patient continues therapy at home. The drugs may be prescribed for him. Masking sounds are also used. Often they are used at night, in order to improve sleep. It can be the sound of a train, the noise of a stream, the sea or the wind.
