Hematology: Chronic lymphocytic leukemia (cll) — Chemotherapy and immunotherapy. Treatment

Chronic lymphocytic leukemia (CLL) features a very slow development. In some cases, the disease does not progress throughout life, so the treatment of patients is not started immediately. The indications for therapy initiation are major changes in the hemogram, enlarged liver and spleen. In recent years, high efficiency in the fight against CLL was show, by targeted drugs.

Diagnostic Measures

On the eve of therapy, the patient undergoes a thorough examination. The physician clarifies the complaints, performs a clinical examination and reviews the case history. The following tests are prescribed:

  • complete blood count,
  • blood chemistry,
  • cytogenetic analysis.

According to the test findings, the optimal regimen is developed.

Therapy

The patient is hospitalized for 14 days. Tyrosine kinase inhibitors (imatinib, dasatinib), which selectively inhibit the division of tumor cells, are used for treatment. The targeted drugs almost do not affect the healthy blood cells, and thus do not cause serious side effects. In rare cases, therapy may be accompanied by nausea, muscle pain, and allergic reactions.

The patient’s condition is continuously monitored by the clinic staff. If the patient feels unwell, s/he is provided with required assistance.

The drugs are taken in pill form. The cost of medication is determined by the cycle intensity. The treatment efficacy is evaluated by:

  • complete blood count,
  • cytogenetic analysis,
  • molecular genetic studies.

Alpha-interferon is also used for treatment of hair cell CLL, which helps to slow down the cancer development significantly.

On the eve of discharge, the oncologist gives another piece of advice. The professional examines the patient and provides recommendations for further treatment.
