About the department

The Department of Adult and Pediatric Urology, Andrology at the Urology Clinic Frechen-Koenigsdorf provides the full range of medical services in its areas of specialization. The department offers medical care to women and men with diseases of the urinary system and men with pathologies of the reproductive system. The department's team of urologists also treats men with andrological diseases. They provide consultations, diagnosis, and treatment for erectile dysfunction, male infertility, and andropause (male menopause). The area of expertise of doctors also includes the treatment of urological diseases in children, in particular enuresis and genital malformations in boys. The specialists often treat oncological diseases, such as prostate cancer, testicular cancer, kidney cancer, and bladder cancer. Of particular interest is the treatment of benign prostatic hyperplasia using holmium laser enucleation (HoLEP). It is worth mentioning that the department is one of the leading medical facilities in Europe in the field of this procedure. Only 10 clinics in Germany offer HoLEP treatment, and the department's team of urologists is one of the most experienced in this field. The department's physicians focus on providing patients with individualized medical care in accordance with the requirements of the German Society of Urology (DGU) and the German Cancer Society (DKG).

The department is headed by Prof. Dr. med. Horst Schuldes. The specialist has more than 30 years of successful clinical practice and has exceptional experience in the treatment of benign and malignant urological diseases. Prof. Schuldes enjoys a reputation as one of the best experts in the treatment of benign prostatic hyperplasia with the innovative HoLEP method. He is also a Member of the German Society of Urology (DGU), the Berlin Society of Urology (BUG), and the German Cancer Society (DKG).

The primary focus of the department's urological team is the treatment of prostate adenoma using holmium laser enucleation (HoLEP). The medical facility is one of the most competent in Germany in the field of the innovative HoLEP procedure, and only 10 urology clinics in the country work with this technique. HoLEP requires expensive equipment and additional training. The department's team of urologists has invaluable experience with HoLEP and regularly demonstrates excellent treatment success rates.

Benign prostatic hyperplasia is an abnormal overgrowth of prostate tissue. The main manifestations of the disease include difficulty urinating, frequent urges to urinate, a burning sensation when urinating, and a scanty amount of urine. Not so long ago, the only effective treatment for an advanced stage of prostate adenoma was traumatic surgery, but the advent of the HoLEP procedure was a real revolution in the world of urology. Holmium laser enucleation of the prostate is a minimally invasive procedure that is performed through a transurethral approach without opening the abdominal cavity. For complete removal of an enlarged prostate, the department's urologists use a holmium laser, which allows for the preservation of the integrity of the external capsule of the organ. Prostate tissues are crushed and moved into the bladder with the help of a morcellator (a special instrument), which enables sampling biological material for a subsequent histological examination (there is no such possibility when using the Greenlight procedure and diode laser treatment). In a relatively short period of time, the HoLEP procedure has gained high popularity worldwide. The effectiveness of the method is comparable to open surgery, but it is virtually bloodless, eliminates serious surgical risks, and is successfully used to treat prostate adenomas of any size. Due to the low trauma of treatment with the HoLEP method, postoperative recovery time is significantly reduced. Immediately after the HoLEP laser procedure, the patient feels a significant relief.

The department's specialists are deservedly proud of their results in the treatment of oncological diseases of the urological spectrum. The doctors most often diagnose and treat men with prostate cancer, which is one of the most common oncological diseases worldwide. The medical facility also attaches great importance to prostate cancer prevention. Screening for prostate cancer includes a PSA test (over 8,000 tests annually) and 3D ultrasound scans. The first-line treatment for prostate malignancies is surgery, which is performed using minimally invasive techniques whenever possible. The department's urologists also specialize in systemic treatment of prostate cancer using chemotherapy, hormone therapy, and targeted therapy. A treatment regimen is prescribed on an individual basis, taking into account the patient's clinical data. Men with end-stage prostate cancer receive palliative care aimed at relieving symptoms and improving their quality of life.

The department's range of services includes effective methods for treating urinary incontinence, or involuntary urination. The disease is most commonly diagnosed in women. This is due to the structural features of their genitourinary system and other factors. There is stress, urgency, situational, mixed, and nocturnal (enuresis) urinary incontinence. Treatment tactics largely depend on the type of urinary incontinence, the reasons for its development, and the severity of symptoms. At the early stages of the pathology, the department's urologists use only conservative methods. Doctors prescribe a course of drug therapy and may recommend patients undergo pelvic floor muscle training, biofeedback, and the use of pessaries. For the treatment of urge incontinence, the department successfully performs botulinum toxin injections. Surgery is the last-line treatment, and the gold standard is the TVT sling procedure. This surgical intervention is classified as low-traumatic and is performed under local anesthesia.

Andrology is an integral part of the clinical practice of the department. It provides qualified medical care for men with erectile dysfunction, infertility, andropause (male menopause), and Peyronie's disease. The department performs vasectomy, a surgical procedure for sterilization in which the vas deferens is ligated.

The department's range of medical services includes the following:
