Hematology:
Chronic myelogenous leukemia (cml) — Chemotherapy and immunotherapy. Treatment abroad

The fight against chronic myeloid leukemia (CML) is started after diagnostics. A suitable method of treatment is selected with regard to the stage of pathology development and general condition of the patient. At the initial phase of CML, the use of targeted (directed molecular) drugs is beneficial.

Diagnostic Measures

The treatment is preceded by the oncologist’s advice. The physician examines the patient and collects his/her case history. To get a complete picture of the disease, the following tests are prescribed:

  • complete blood count,
  • blood chemistry,
  • cytogenetic analysis.

According to the test findings, the optimal regimen is developed.

Therapy

The patient is hospitalized for 21 days for medical treatment. In the clinic, s/he is monitored by the medical staff continuously and takes tests to evaluate the therapy effectiveness. The cost of medication is determined by the treatment intensity.

As regards drugs, the tyrosine kinase inhibitors (TKI) are preferred. They selectively block the division of leukemia cells with the Philadelphia chromosome, while not causing any harm to healthy cells. Unlike conventional chemotherapeutic agents, TKI feature a low toxicity and do not cause serious side effects. Their use allows achieving remission to 95% patients.

Imatinib is recognized as the primary therapy. If it proves ineffective, the patient is transferred to dasatinib. Both drugs are administered orally. The treatment results are evaluated by:

  • complete blood count,
  • cytogenetic analysis,
  • molecular genetic studies.

If the patient does not respond to any tyrosine kinase inhibitor, s/he is prescribed interferon-alpha (sometimes in combination with cytostatics). If successful, this therapy can significantly slow down the disease progression.

Upon cycle completion, the tests are carried out. The patient talks to the oncologist, who provides recommendations for further treatment.
