Pelvic organ prolapse refers to the descent of one or more pelvic organs. Prolapse can be anterior, middle, or posterior. These types of pelvic organ prolapse can be treated with anterior or posterior colporrhaphy. Less commonly, physicians place a mesh between the vagina and the rectum or repair a rectocele through the rectum. Cystocele surgery

 

In the case of cystocele, plastic reconstruction of the pelvic organs is usually performed through a vaginal approach. Basic operations are as follows:

Anterior colporrhaphy. Doctors shrink the anterior vaginal wall with sutures. This is a safe operation with a low risk of complications. Many patients require a single-stage plastic surgery of the middle part of the pelvic floor. When using this stage of the operation, doctors manage to achieve more stable results in cystocele treatment.

Colposuspension. This surgical procedure is carried out in cases where a cystocele is combined with a urethrocele (urethral prolapse) and insufficiency of the sphincter that retains urine. The essence of the operation is to bring the vaginal fascia closer to the pelvic wall on each side of the neck and the bottom of the bladder with fixation by applying sutures through the sacroiliac ligament.

In the case of concomitant urinary incontinence, a sling procedure can also be used, namely the implantation free synthetic loop. Doctors at German centers use a mini-sling (single-incision mini-sling) for the implantation of which a single incision on the anterior vaginal wall is made.

Rectocele resection

 

There are three types of operations that can be performed to treat the rectocele. Depending on the surgical approach, they can be divided into surgical procedures performed:

  • through abdominal incisions;
  • through the vagina;
  • through the anus.

In recent years, transanal operations have become less and less common in Germany. This is because the results are less reliable as compared to other types of interventions. The main disadvantage of the method is that it causes pain during intercourse after surgery, which makes this type of treatment unsuitable for women who continue to have their intimate life.

A modified sacrocolpopexy is performed simultaneously through the abdomen and vagina. Doctors in Germany prefer a minimally invasive laparoscopic approach. They treat the rectocele by placing a mesh between the vagina and the rectum. It is undesirable to use this technique on women who are sexually active, as sexual intercourse can become painful. The placement of a mesh implant can also lead to complications caused by the presence of a foreign body. With all the shortcomings of this operation, doctors still sometimes perform it because it is very effective and reliable, even in the case of severe rectocele.

Most women undergo a posterior colporrhaphy. This is a reliable operation that eliminates symptoms and provides long-term results. At the same time, it is minimally traumatic and does not require long-term rehabilitation. The intervention is performed through the vagina. Its essence lies in the suturing of the posterior vaginal wall.

Some women require treatment for both cystocele and rectocele. In this case, anterior and posterior colporrhaphy can be performed. It can also be supplemented with plastic surgery on the middle part of the pelvic floor, if necessary.
