What is HoLEP for prostate adenoma?

 

HoLEP uses a holmium laser. It delivers radiation through flexible quartz fibers and has a wavelength of 2100-2140 nm with a penetration depth of 0.5 mm. The chromophore (absorbing substance) for this type of laser is water.

The shallow penetration depth ensures the safety of the procedure. Holmium laser enucleation of the prostate, HoLEP, is performed without thermal damage to the tissues surrounding the treatment area.

HoLEP is performed under general anesthesia. The procedure is performed through the urethra without any abdominal incisions. Doctors use a laser to dissect the tissue. In the original version of HoLEP, it starts with the excision of the prostate at 5 and 7 o'clock on the conventional dial. This is followed by the enucleation of the median lobe of the prostate gland. Today, doctors more often use a modified two-lobe enucleation technique. They excise the tissue, coagulate the blood vessels to stop bleeding, and then crush the resected tissue with a morcellator and remove it from the bladder.

Success story: how HoLEP became the best procedure for the treatment of prostate adenoma?

 

The holmium laser was first used for the treatment of benign prostatic hyperplasia (BPH) in 1994. However, at that time, the technique used was vaporization and incision, not enucleation, which allows for almost complete removal of the prostate. The HoLEP technique came about after the development of the mechanical morcellator. This is a tube into which prostatic tissue is sucked, crushed, and suctioned through the urethra to the outside environment.

Already in 1998, the results of the first HoLEP study in patients with a prostate volume of 75.5 ml were published. The same authors later published the results of a seven-year follow-up of patients, confirming the excellent long-term results of HoLEP treatment for benign prostatic hyperplasia.

In 2000, the results of a study comparing HoLEP with open adenomectomy, a conventional prostate removal surgery for prostate volumes greater than 100 ml (large prostate), were published. The treatment outcomes were similar, and the risk of complications was much lower in the HoLEP group.

In 2007, HoLEP was first proven to be superior to open surgery, and soon after, HoLEP was confirmed to be effective for prostate volumes greater than 200 mL. Since then, there is no longer a limit on the volume of benign hyperplasia for the HoLEP procedure in urology.

A comparison between HoLEP and TURP (transurethral resection of the prostate) showed that HoLEP provides better results. The risk of complications is the same. The duration of bladder catheterization, hospital stay, and temporary dysuria (urinary dysfunction) are shorter after TURP.

Who may be a candidate for HoLEP for the treatment of prostate adenoma?

 

Holmium laser enucleation can technically be performed on all patients, regardless of prostate size. In practice, however, the procedure is more commonly used in patients with a large prostate volume of more than 80 ml, while minimally invasive ablative procedures or TURP are used for prostates smaller than 80 ml.

The HoLEP procedure is considered more of an alternative to surgery through abdominal incisions than a replacement for TURP. By choosing HoLEP over surgery, patients can expect the same treatment outcomes, but with a more comfortable and shorter recovery and a lower risk of complications.

Advantages and disadvantages of HoLEP for prostate adenoma

 

With many advantages, HoLEP surgery has almost no disadvantages. This technique:

  • Allows for the removal of prostatic hyperplasia (BPH) of any size
  • Does not require any abdominal incisions
  • Provides an effect at least as good as that of open surgery and TUR
  • Provides good long-term results with virtually no risk of prostate adenoma recurrence (except in cases of incomplete removal)
  • Offers a low risk of complications compared to conventional surgery
  • Provides the same risk of complications as TURP, but HoLEP is more effective
  • Minimal blood loss due to the instantaneous coagulation of blood vessels during laser dissection

 

The disadvantages of holmium laser enucleation for prostate adenoma are few and insignificant.

The procedure takes longer than TURP. However, as a result of its longer duration, the risk of TUR syndrome due to fluid absorption into the systemic bloodstream does not increase because the laser coagulates (closes) the blood vessels through which saline resorption occurs during tissue dissection.

Compared to TURP, patients require longer bladder catheterization and have longer hospital stays.

Safety of HoLEP for prostate adenoma

 

HoLEP provides a complete cure for even the most severe forms of benign prostatic hyperplasia (BPH) and is very safe. Most complications are short-term and resolve within 2-3 weeks.

The most common complication is transient (temporary) urinary incontinence, which develops in 11% of patients. The incidence of bladder neck sclerosis is 6% and urethral stricture is 1.3%, which is comparable or even lower compared to TURP. This is despite the fact that HoLEP is performed in more severe cases, usually when the prostate volume exceeds 80 ml.

The risk of serious complications is less than 1%. These include bleeding, infection, bladder injury, and the conversion of HoLEP to open surgery.

HoLEP causes retrograde ejaculation in most patients and may also impair erectile function. However, the risk of these complications is comparable to TURP and open surgery.

The safety of the surgical procedure is influenced by the experience of the surgeon. The lowest risk of complications is observed when the operation is performed by a surgeon who has performed at least 50 such surgical interventions.
