Ligamentotomy and autologous tissue grafting are commonly used for the enlargement of the size of the normal penis. With these operations, doctors can increase both the length and width of the penis without losing its function and sensitivity.

Ligamentotomy is a simple operation that began to be used at the end of the 20th century. The essence of the surgical technique is to eliminate the curvature of the penis by dissecting the suspensory ligament. The cavernous bodies will then be fixed in a new position. This operation is usually supplemented with a V-Y rotation advancement flap plastic surgery above the pubis. The technique increases the length but not the width of the penis.

The following options can be used to increase the width of the penis:

  • free autologous fat injections (this approach is rarely used due to a high risk of complications);
  • microsurgical transplantation of the patient's own tissues;
  • transposition (rotation) of vascularized (having blood supply) muscle or fat fascial grafts.

Microsurgical autotransplantation is an operation during which a doctor wraps the cavernous bodies of the penis with the patient's own tissues. This is followed by a microsurgical stage of the operation to restore the blood supply to the implanted flap. Many variants of autotransplantation have been developed depending on the tissues used. For example, these can be sections of the latissimus dorsi muscle, the serratus anterior muscle or its fascia, a section of the rectus abdominis muscle, and a fat fascial flap of the anterior abdominal wall.

The operation is complex, requiring great experience and skills from a team of surgeons. If unsuccessful, the anastomoses, which should provide blood supply to the transplanted tissues, may fail or become blocked with blood clots.

Artificial tissue implantation (for example, silicone). The advantages of this method are that it is not necessary to inflict additional trauma in the donor area. The operation is easier as the artificial materials do not need any blood supply. The technique, however, is not widely used due to a high risk of infectious complications. A promising method is the implantation of biopolymer foam under the skin, which triggers the growth of the penis' own tissues.

Phalloplasty for hypoplasia

 

Phalloplasty is actually the creation of a new penis from other tissues. This is an extremely complex reconstructive operation, but surgeons from the hospitals in Germany perform it with good results.

When performing phalloplasty, some or all of the goals from the list below must be achieved:

  • penis enlargement;
  • ensuring tactile sensations;
  • restoring the erectile mechanism;
  • an ability to urinate while standing;

Each stage of the operation can be performed using many different techniques, and this is especially true for the formation of the penis and urethra.

The choice of surgical tactics depends on the patient's wishes. For example, on how important it is for him to urinate while standing. If it is important, additional urethral lengthening may be necessary. This is a complex stage of the operation, increasing the risk of complications. If a patient agrees to sedentary urination, doctors can only enlarge the shaft of the penis, and this is easier and safer.

Doctors use tissue from other parts of the human body to repair penile tissue. Historically, the first technique was the use of local pedicled skin flaps to preserve the blood supply. However, with the advent of microsurgical techniques, anatomical proximity has ceased to be important. Doctors can now use tissues from any part of the body. When they are transplanted, surgeons suture the blood vessels together, ensuring good vascularization of the implanted tissues.
