  • transposition (rotation) of vascularized (having blood supply) muscle or fat fascial grafts.

Microsurgical autotransplantation is an operation during which a doctor wraps the cavernous bodies of the penis with the patient's own tissues. This is followed by a microsurgical stage of the operation to restore the blood supply to the implanted flap. Many variants of autotransplantation have been developed depending on the tissues used. For example, these can be sections of the latissimus dorsi muscle, the serratus anterior muscle or its fascia, a section of the rectus abdominis muscle, and a fat fascial flap of the anterior abdominal wall.

The operation is complex, requiring great experience and skills from a team of surgeons. If unsuccessful, the anastomoses, which should provide blood supply to the transplanted tissues, may fail or become blocked with blood clots.

Artificial tissue implantation (for example, silicone). The advantages of this method are that it is not necessary to inflict additional trauma in the donor area. The operation is easier as the artificial materials do not need any blood supply. The technique, however, is not widely used due to a high risk of infectious complications. A promising method is the implantation of biopolymer foam under the skin, which triggers the growth of the penis' own tissues.

Phalloplasty for hypoplasia

 

Phalloplasty is actually the creation of a new penis from other tissues. This is an extremely complex reconstructive operation, but surgeons from the hospitals in Germany perform it with good results.

If it is important, additional urethral lengthening may be necessary. This is a complex stage of the operation, increasing the risk of complications. Doctors use tissue from other parts of the human body to repair penile tissue. Historically, the first technique was the use of local pedicled skin flaps to preserve the blood supply.
