What is the process of treatment for advanced stages of duodenal cancer?

 

In the vast majority of cases, duodenal cancer is detected in its advanced stages. This is due to the fact that small neoplasms do not cause any symptoms, and by the time they are detected, the tumor has already spread within the abdominal cavity.

The duodenum is located at the "intersection" of the digestive system. It is connected to the stomach and colon and surrounds the pancreas. Bile and pancreatic ducts flow into it. The surgical removal of the tumor is therefore complex and time-consuming. The success of the procedure varies from hospital to hospital. Much better results with a lower risk of complications are achieved at specialized centers in countries offering advanced medical services.

In the advanced stages of colon cancer, cytoreductive surgery with subsequent hyperthermic intraperitoneal chemotherapy is considered the best treatment option. The doctor first removes all tumor foci and then rinses the abdomen with a heated solution of cytostatics to eliminate the remaining cancer cells.

What are cytoreductive surgery and HIPEC for duodenal cancer?

 

It is believed that once the tumor has spread to the abdomen, it is impossible to completely remove it. Therefore, any surgical procedures performed in these cases are considered cytoreductive (aimed at shrinking tumor foci) rather than radical (potentially curing). As a rule, these are complex surgical interventions lasting many hours because doctors need to thoroughly examine the abdominal cavity and remove all the organs and tissues affected by the tumor.

When performing surgery for duodenal cancer, doctors may perform the total or partial removal of multiple organs. They remove parts of the small intestine, stomach, pancreas, gallbladder, bile ducts, spleen, the affected parts of the peritoneum, lymph nodes, greater omentum, and lesser omentum. They may sometimes also remove pelvic organs such as the ovaries, uterus, part of the sigmoid, rectum, bladder, etc. The extent of the operation will depend on the tumor's spread.

It is carried out immediately after tumor removal, at the final stage of the surgical intervention. The abdominal cavity is rinsed through the drainage system with a heated solution of cytostatics. The effectiveness of the procedure is also increased due to the heating of the solution to 39-43 degrees. At this temperature, the cytotoxic properties of drugs are enhanced, and the solution penetrates into the tumor foci to a depth of 2-3 mm.

As a rule, the perfusion lasts about an hour. The wound is then tightly sutured, thereby completing the operation.

The effectiveness of CRS and HIPEC in Germany for duodenal cancer

 

So far, there are no statistics on the survival rates achieved with this technique. This is because duodenal cancer is very rare, and not all patients with this cancer have peritoneal carcinomatosis. Even if it develops, most countries and hospitals do not treat these patients with HIPEC because the procedure is mainly used in countries with advanced healthcare systems.
