What are the types of optic neuropathies
Optic neuropathies are divided into acquired and inherited ones. Acquired neuropathies affect people of all ages. Damage often affects not only the optic disc, but also the entire optic nerve, blood vessels and even accompanied by intracranial diseases.
The origin of ONA is diverse. The causes are vascular, inflammatory, compression (i.e. nerve compression), infiltrative, traumatic, toxic and alimentary.
Hereditary optic neuropathies develop in childhood. The main pathologies are Leber hereditary optic neuropathy (LHON) and autosomal dominant optic atrophy (ADOA). There are others that are less common. In optic nerve hypoplasia, the child is born with fewer axons in the neurons structure, resulting in low visual acuity.
Glaucoma, one of the most common causes of optic atrophy, is also considered a genetic disease, although it develops as a result of the interaction of multiple genes with environmental factors.
However, all cases of optic atrophy have common consequences: death of retinal ganglion cells and loss of axons. It is their restoration that cell therapy is aimed at. To enhance regeneration, SCs are used. These are immature cells with the ability to self-renew and differentiate into mature cells.
How the treatment is carried out
So far, the stem cell therapy for optic nerve atrophy in Germany is carried out differently in different medical centers. The types of SCs, methods of obtaining them, and the routes of administration differ.
Most often SCs are obtained from the patient's blood, less often from bone marrow or adipose tissue, which is a good source of mesenchymal SCs. They are then injected into the eye (retrobulbar, subtenon and intravitreal injections), more rarely intravenously. If the patient is receiving surgical treatment, it is possible to inject stem cells directly into the optic nerve area.
The studies show good results of SCs application in optic nerve atrophy. As a result of treatment, vision improves in a few weeks and further progression of the pathology is slowed down. The achieved improvements are maintained even 12 months after the treatment completion. Stem cell transplant for ONA in Germany can be repeated to maintain and improve the therapeutic results.
