About the disease

Parotid gland cancer is subtype of salivary gland cancer, which develops in front of the ear. Overall, salivary glands consist of parotid, submandibular and sublingual glands, which are located inside cheeks, mouth, throat, and ear. Parotid gland cancer has the highest incidence among these subtypes.

In most cases this type of cancer develops if there were mutations or transformations in DNA of salivary gland. Such mutations form neoplastic cells, which then start to grow and spread creating cancer. If not diagnosed on time, parotid gland cancer can spread to other organs, including distant organs of body.

Such DNA mutations can develop either because of older age or, more often, radiation exposure, when a person is exposed to harmful toxic substances. In rare cases parotid gland cancer can appear as a result of head cancers, when patient underwent several courses of radiation frequency and did not fully recover from exposure to radio rays.

According to Cancer Research UK, 5-year survival statistic for this type of cancer, around 90% of patients survive if this type of cancer was treated during the first stage. During the second stage 75% of patients survive. During the third stage 65% survive. In the fourth stage 40% survive.

Symptoms

  • Lumpy swelling close to ear, mouth and jaw
  • Partial facial numbness
  • Facial muscle weakness on the affected side of face
  • Pain close to the ears
  • Problems with swallowing and opening mouth widely
  • Lack of appetite

Diagnosis

First of all a doctor performs physical test to feel patient's jaw, cheeks, area around the ears to determine if patient has swelling or unpleasant sensations.

  • Magnetic resonance imaging can determine exact location of cancer, its size and stage of progression.
  • Biopsy is also an effective diagnostic method during which a doctor takes a sample tissue from the place where cancer is most likely to be located.
  • Computerized tomography also determines location and extent of parotid gland cancer.

Treatment

In most cases patients need to undergo surgical resection as parotid gland cancer treatment. During surgical resection doctor removes tumor and part of the healthy tissue. Such partial resection can be performed in beginning stages of cancer, when it did not spread to other organs and did not affect other subtypes of salivary glands.

If cancer is big in its size, doctor can remove not only parotid gland, but all salivary gland and in some cases facial nerves.

If left untreated, parotid gland cancer first spreads to lymph nodes. Such metastases usually occur during the third stage. In that case surgical resection can be accompanied by neck dissection, which is a removal of lymph nodes. 

After surgical resection doctors can also perform reconstructive surgery, which is necessary if there was resection of certain bones, skin and nerve endings. In some cases doctors use grafts from other parts of patient's body to reconstruct facial features and restore chewing, speaking, swallowing functions.

Surgical resection can be combined with radiation therapy and chemotherapy.
