Rehabilitation principles

 

The first case of cardiac rehabilitation in history was described in 1772. W. Heberden reported in his publication about the improvement in the condition of a patient who sawed wood for half an hour a day. This was the first evidence that physical activity after heart disease should not be limited but encouraged.

A person who suffered a heart attack was forced to lie in bed for three weeks at the end of the twentieth century. Today, however, approaches have changed as convincing evidence has been obtained confirming the need for the patient's early activation.

Physical activity is at the basis of any cardiac rehabilitation program, and other methods are of secondary importance. They create favorable conditions for achieving a good result through exercise and improving the patient's condition.

Indications and contraindications

 

The main indications for cardiac rehabilitation are the following:

  • previous myocardial infarction;
  • stable angina;
  • previous heart surgery, such as coronary artery bypass surgery, heart valve repair or replacement, and a heart transplant;
  • previous endovascular procedures, such as stent implantation and angioplasty, and minimally invasive interventions on heart valves;
  • atherosclerosis;
  • heart failure of 2-3 functional classes.

Contraindications for rehabilitation are severe decompensated heart failure, unstable angina, severe arrhythmias, pulmonary hypertension, the presence of a thrombus in the heart, obstructive cardiomyopathy, and severe aortic valve stenosis.

Stages of rehabilitation

 

Cardiac rehabilitation takes place in the following three stages:

  • phase 1 begins immediately after the disease or surgery in the Intensive Care Unit;
  • phase 2 takes place on an inpatient basis;
  • phase 3 takes place on an outpatient basis with regular visits to a Rehabilitation Center.

Rehabilitation includes exercise, lifestyle modifications (including diet), and drug therapy (normalization of blood pressure and cholesterol levels).

Types of workouts include the following:

  • running;
  • walking;
  • exercising on cardiovascular equipment;
  • cardiovascular exercise without any special equipment, such as cycling, skiing, playing sports, etc.;
  • exercising in the water.

Exercise aims to reduce weight and train the heart, which becomes able to endure more intense loads.

It is important that the workout is controlled. Cardiac rehabilitation in Germany is carried out with the participation of a team of professionals, such as rehabilitation specialists, highly specialized doctors, cardiologists, physiotherapists, massage therapists, exercise therapy instructors, etc. The program is compiled individually and regularly adjusted, taking into account the state of the cardiovascular system and the musculoskeletal system, the initial load tolerance, and the results achieved during training.
