Malignant tumors located in the pelvic organs may spread to the peritoneum. In this case, patients have multiple metastases that are not well controlled with systemic therapy. In such severe cases, cytoreductive surgery comes to the rescue. This is a complex operation to remove multiple tumors in the abdominal cavity, but its successful performance allows doctors to achieve long-term remission of the disease. It increases the life expectancy of patients by several years. Doctors at modern Oncology Centers usually complement cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (HIPEC).

What is cytoreductive surgery?

 

Cytoreductive surgery is the general term for operations aimed at reducing the tumor foci in the body as much as possible.

These surgical procedures are performed for multiple tumor foci in the same anatomical area, most often in the peritoneum, which covers the abdominal and pelvic organs.

The best results are achieved when all tumor foci can be removed. Such a cytoreduction is called a complete cytoreduction. It provides patients with good long-term survival rates. In leading hospitals in Germany, the frequency of achieving complete cytoreduction reaches 90% of all operations performed.

When can cytoreductive surgery be used?

 

Cytoreductive surgery is most often used to treat cancer that has spread to the peritoneum. It is used if radical (potentially curing) surgery cannot be performed. Cytoreductive surgery thus refers to methods of treating stage III-IV cancer.

The primary cancers for which cytoreductive surgery is used include ovarian cancer, colorectal cancer, small intestine cancer, large intestine cancer, stomach cancer, appendix cancer, and primary peritoneal tumors. Such surgical interventions are less often performed for duodenal cancer, gallbladder cancer, pancreatic cancer, and liver cancer.

Candidates for cytoreductive surgery are patients who can tolerate complex traumatic surgery, have a low peritoneal cancer index, and do not have any distant metastases outside the abdominal cavity.

How is cytoreductive surgery performed?

 

There is no generally accepted technique for performing cytoreductive surgery. It is a general term for surgical interventions of different types. They have only one common goal: maximum shrinkage of the tumor mass. The specific organs that the surgeon will remove, the need for additional stages of the surgical procedure, and other factors depend on the type of cancer, its location, size, and number of tumors.

A large study involving more than a thousand patients who underwent cytoreductive interventions showed that, on average, two organs and four sections of the peritoneum are removed in one person. Soft tissue and lymph nodes involved in the malignant process are also often removed.

In most cases, the surgery is performed through a long incision in the anterior abdominal wall (laparotomy). As a rule, it lasts 5-6 hours, although the duration can vary significantly from patient to patient.

After creating a surgical approach, doctors thoroughly examine the abdominal cavity to find out and remove all tumor foci. As a rule, the organs in which the primary tumor appeared are totally or partially removed. For example, for ovarian cancer, the uterus and appendages are removed, and for colorectal cancer, intestinal resection is performed. Adjacent organs involved in the cancer process are also often partially or totally removed.

Many patients require a reconstructive stage of the operation, such as suturing the intestine after resection to restore its continuity, creating a urinary bladder from segments of the small intestine, etc., depending on the organs removed. In any case, doctors strive to achieve maximum restoration of the functions of the affected organs and systems to improve the quality of life of the patient.

Leading hospitals worldwide have begun to perform laparoscopic cytoreductive surgery. The possibility of minimally invasive treatment is being explored for colon and appendix cancer, but only in patients with a low peritoneal cancer index. The initial results are promising because doctors have been able to achieve survival rates comparable to open surgery with a low risk of complications and a relatively short recovery period. The laparoscopic technique allows for intraoperative hyperthermic intraperitoneal chemotherapy to destroy the remaining cancer cells, if necessary. So far, this approach to cytoreductive surgery is not considered standard. It is practiced only in some specialized Oncology Centers.
