Liver cancer is still one of the most severe types of cancer. However, German surgeons are achieving phenomenal results in its treatment. The five-year survival rate for patients undergoing extensive liver resection is as high as 70-80% in some cancer centers. German surgeons are increasingly using minimally invasive techniques to remove tumors. These surgical procedures are just as effective as open surgery, but safer.

When can surgery be performed?

Whenever possible, the doctor treats liver cancer with a sparing resection. However, when this cannot be done, an extensive resection is the option of choice. It is required when the tumor is too large, has grown into neighboring anatomical structures, or has metastasized to the nearest lymph nodes.

At the same time, there are many limitations that make extensive liver resection for metastases impractical. German surgeons carefully select patients for this operation. The surgical tumor removal can be performed if:

  • there is no cirrhosis or the disease is not severe (Child A);
  • impairment of the absorption and excretion function of the liver is stage 2 at most (determined by radioisotope diagnostics);
  • the size of the parenchyma after surgery is not less than 30%, otherwise liver failure may develop;
  • in the case of cirrhosis, it is better if at least 50% of the parenchyma remains.

The doctor thoroughly examines the patient prior to the operation. A CT or MRI scan or angiography may be performed. Laboratory tests are done to assess liver function. The examination is required both for planning the operation and for detecting possible limitations and contraindications to it.

The essence of the operation

During the operation, the doctor removes several segments of the liver. In addition, the kidney, adrenal gland, pancreas, blood vessels, lymph nodes, and other organs that may be involved in the pathological process may be resected or removed. The operation is most often performed using an open approach, while minimally invasive procedures are less common.

Minimal invasive treatment

German surgeons at the best cancer centers perform liver tumor removal during minimally invasive surgery. Laparoscopic liver resection, hybrid, and robot-assisted surgeries are used.

Minimally invasive surgical procedures for malignant liver tumors have not yet become widespread worldwide. Some clinics perform laparoscopic resection of a segment of the liver for small neoplasms with a marginal location. Only a few highly specialized cancer centers perform laparoscopic extensive liver resections. They report results that are equal to or even better than those of classic open surgery. The main advantage of this technique is the low risk of complications, low intraoperative and postoperative mortality, and a relatively short rehabilitation period.

Tranchart et al. compared the results of open and laparoscopic surgery for liver cancer. They analyzed the one-year, three-year, and five-year survival rates. It turned out that they were the same. The risk of recurrence was also comparable. The complication rate associated with the surgical removal of part of the liver was significantly lower after laparoscopic surgery.
