When can surgery be performed?

 

Whenever possible, the doctor treats liver cancer with a sparing resection. However, when this cannot be done, an extensive resection is the option of choice. It is required when the tumor is too large, has grown into neighboring anatomical structures, or has metastasized to the nearest lymph nodes.

At the same time, there are many limitations that make extensive liver resection for metastases impractical. German surgeons carefully select patients for this operation. The surgical tumor removal can be performed if:

  • there is no cirrhosis or the disease is not severe (Child A);
  • impairment of the absorption and excretion function of the liver is stage 2 at most (determined by radioisotope diagnostics);
  • the size of the parenchyma after surgery is not less than 30%, otherwise liver failure may develop;
  • in the case of cirrhosis, it is better if at least 50% of the parenchyma remains.

The doctor thoroughly examines the patient prior to the operation. A CT or MRI scan or angiography may be performed. Laboratory tests are done to assess liver function. The examination is required both for planning the operation and for detecting possible limitations and contraindications to it.
