Overview
Ovarian cancer is cancer that affects ovarian tissue. The disease occurs due to the formation of a tumor in the ovary, and can also develop due to metastatic lesions when malignant cells spread from a tumor located in another organ. The danger of ovarian cancer is in the absence of symptoms, and therefore it is almost impossible to reveal cancer at an early stage.
There are three main types of ovarian malignant tumors. These are epithelial cell tumors, germ cell tumors, and stromal tumors. Epithelial cell tumors are by far the most common ones, while stromal tumors account for less than 1% of all ovarian cancer cases.
Several risk factors can increase the risk of developing ovarian cancer. These include both modifiable factors and factors that the patient can not influence. The first of non-modifiable risk factors is age. Ovarian cancer is rare in women under 40, and about half of all ovarian cancer cases are diagnosed in women aged 63 and older.
According to medical practice, if a tumor is confined to the inside of the ovary, there are no difficulties with its treatment or removal, and additional therapy is not required after the surgery.
In oncology practice, doctors note that patients with early stage malignant tumors have more chances of recovery. It is during this period that there is an opportunity to conduct surgery or chemotherapy to destroy all cancer cells. But the main problem lies in the fact that often ovarian cancer is asymptomatic, and it is not always possible to detect the disease before its progression. The only way to diagnose ovarian cancer at an early stage is to undergo annual preventive check-ups.
There is also a connection between obesity and ovarian cancer development, with overweight women at a much higher risk of the disease. Also, reproductive history is a risk factor. Women who got pregnant before the age of 26 have a lower risk of developing ovarian cancer. Women who do not have a pregnancy before or after 35 years of age are at a higher risk of ovarian cancer.
Other risk factors of ovarian cancer include taking birth control pills, having surgery to ligate the fallopian tubes (tubal ligation), or hysterectomy. Hormone replacement therapy increases the risk of disease as well. As with many other cancers, family history is important.
Ovarian cancer symptoms and stages
Some of the symptoms of ovarian cancer include:
- Abdominal pain, bloating, enlargement of the abdomen
- Pelvic pain, back pain
- Nausea and vomiting
- Fatigue
- Pain during sexual intercourse
- Frequent urination and urgency to urinate
- Menstrual irregularities
In the case of metastases in the lungs, at the advanced stages, cardiovascular and respiratory failure arise, edema of the lower extremities, and thrombosis develop.
There are four main stages of ovarian cancer, as well as many substages, which are classified according to the degree of cancer spreading. The main stages include:
- First-stage cancer is confined to the inside of the ovary, without spreading to lymph nodes or other organs
- Second-stage cancer spreads to at least one pelvic organ, such as the bladder or uterus, but not affect the lymph nodes
- Third-stage cancer spreads to the abdominal organs, lymph nodes of the abdomen, or both
- Fourth-stage cancer spreads outside the abdomen to organs located elsewhere, such as the lungs
The diagnosis of ovarian cancer is usually confirmed with a combination of physical examination and diagnostic imaging, such as ultrasound scanning, X-ray, MRI, and computed tomography. Biopsy and exploratory surgery, known as laparotomy, can be used to confirm the diagnosis or to determine the stage of cancer.
Treatment
Success in the treatment of the vast majority of malignant neoplasms depends on the possibility of their radical removal. The absence of metastases gives more chances for the complete recovery. Unfortunately, in many cases, untimely diagnosis or an aggressive progression of oncology makes healthcare personnel start treatment at the stage of metastatic lesions in regional lymph nodes. Sometimes the tumor grows into neighboring organs, involving in the process the important anatomical structures that cannot be removed.
The innovative method of hyperthermic intraperitoneal chemotherapy (HIPEC) allows reaching a completely new level in the effectiveness of the treatment of patients with ovarian cancer metastases. The combined use of cytoreductive surgery and the introduction of cytostatic drugs into the abdominal cavity helps to achieve a high survival rate of ovarian cancer patients.
The high temperature of the solution containing anticancer drugs allows for better tissue penetration, creating the maximum therapeutic concentration of the drug directly in the loci of the pathological process, thereby reducing the likelihood and severity of systemic side effects of treatment.
Hyperthermic intraperitoneal chemotherapy is carried out in several stages.
Primary cytoreductive surgery involves removing as much of the tumor and metastases as possible before starting hyperthermic intraperitoneal chemotherapy. Primary cytoreductive surgery is the standard of care for advanced ovarian cancer, especially of the third stage of the disease. The goal of cytoreductive surgery is complete removal of the tumor or leaving its minimum volume at the end of the operation. This surgery is not indicated for patients with metastases in the liver and lungs.
Intermediate cytoreductive surgery is performed after a short course of chemotherapy (usually 2-3 courses). The operation at this stage is an acceptable approach in the treatment of many patients in whom the first surgery was unsuccessful.
When all visible loci of tumor growth are removed, doctors proceed to the stage of hyperthermic intraperitoneal chemotherapy.
The chemotherapy drug is introduced into the abdominal cavity through a hyperthermoperfuser. This device provides a constant temperature of the supplied drug (usually 41-43 degrees). After the procedure completion, the chemotherapy drug is removed from the abdominal cavity through pre-installed drains. This procedure lasts from half an hour to two hours. To exclude the evaporation of the drug and a decrease in its temperature, the wound is insulated with an airtight material during the perfusion.
At the end of the hyperthermic intraperitoneal chemotherapy session, the surgical wound is sutured, a doctor leaves drainages in it. If the risk of recurrence is high, a second procedure is performed within a week after the first session, but with a chemotherapy drug of normal temperature.
Hyperthermic intraperitoneal chemotherapy in Turkey is carried out in patients suffering from malignant tumors of the abdominal cavity, including metastatic colon cancer, ovarian cancer, stomach cancer, and some other types of oncology. Recently, hyperthermic intraperitoneal chemotherapy has started being applied in patients with secondary foci in the liver. Oncologists determine the possibility of performing hyperthermic intraperitoneal chemotherapy strictly individually. It is important to note that this method is not effective when the disease spreads outside the abdominal cavity.
