It is impossible to provide treatment without the full-fledged diagnostics. Infertility is a multifactorial problem. This is not a single separate disease, but only the state of the body, which means that at the moment the conception of a child is impossible. It is diagnosed if a couple who is not protected for 12 months or more cannot achieve pregnancy.

Since this condition can have many causes, each case of infertility requires the thorough diagnostics. There can often be combined at once 2 or more infertility factors. Infertility can be male and female. Female infertility is only slightly higher than male infertility. In 20% of cases, both representatives of the couple have problems with fertility.

The main female infertility factors are: 

  • tubal-peritoneal factor of infertility, which involves the impaired patency of the fallopian tubes;
  • uterine factor infertility, which involves a change in the shape, size of the uterine cavity or endometrial condition;
  • endocrine factor infertility, which involves hormonal imbalance;
  • cervical factor infertility, which involves the obstruction of the cervical canal.

There are hundreds of diseases and conditions that lead to decreased fertility. Therefore, a thorough examination is required to determine the causes that can lead to infertility.

To do this, a woman should undergo pelvic ultrasound scan, hysteroscopy, hysterosalpingography, hormonal testing, as well as tests to detect sexually transmitted infections, blood coagulation, autoimmune pathologies. Based on the results of these tests, the doctor may prescribe other tests.

It is definitely worth examining male sperm, because a woman can be treated for a long time and without success, but if there are problems on the part of the man, all measures will be unsuccessful.

Treatment of female infertility in Germany 

 

The tubal factor infertility, endocrine factor infertility and cervical factor infertility are most common for women. All of these conditions are treatable.  

The tubal factor infertility means that the egg cannot be delivered from the ovary to the uterus, because even if it can mature, it will need to go through the fallopian tube. If the tube is obstructed, fertilization cannot take place. The obstruction mostly occurs due to the previous infection. whereas in rare cases it can be caused by a surgical procedure. 

There are two options used for the treatment of tubal factor infertility:  

  • surgery aimed at reconstruction of the fallopian tubes;
  • in vitro fertilization (IVF).

In most cases, the doctors start from the first option. However, if pregnancy does not occur after laparoscopy and restoration of the fallopian tube patency, the doctors have to resort to IVF. This procedure involves removing the egg directly from the ovary and the transferring the embryo to the uterus. The fallopian tubes are not involved in this process, so even if they are obstructed or removed, the IVF allows for a successful pregnancy.

The treatment of female infertility in Germany for endocrine causes can be completely different, depending on the identified disorders. There are a huge number of pathological conditions that can disrupt ovulation and the menstrual cycle. This is a violation of the formation of estrogens, progesterone, glucocorticoids, thyroid hormones, gonadotropins, prolactin and many other substances.

There are two main options for solving the problem: 

  1. Correction of the endocrine profile. If there is a lack of hormones, the patient will be prescribed hormone replacement therapy, for example, thyroxine for hypothyroidism or progesterone for luteal phase deficiency. The doctors can also prescribe drugs that lower the levels of certain hormones or block their receptors in order to reduce their effect on the body (an excess of prolactin, male sex hormones, glucocorticoids can often become the cause of impaired egg maturation and lack of ovulation). Once the endocrine profile is corrected, the woman will restore the menstrual cycle and fertility, which allows her to get pregnant. 
  2. Ovulation induction. It is carried out if the hormonal causes of impaired fertility cannot be corrected or therapy does not yield results. The ovulation induction can often be used in case of low ovarian reserve, which can affect older women or occur due to premature ovarian failure. The doctors prescribe the drugs that stimulate the maturation of the egg. This process is ultrasound-guided. Once follicles get mature, an ovulation trigger will be introduced. This process will be followed by a fertilizing intercourse. 

In the case of the uterine factor infertility, the causes of infertility can be very diverse, for example, infectious endometrial lesions, polyps and synechiae in the uterus, large myomatous nodes, foci of endometriosis, etc. The congenital anatomical defects can also occur. The problem in this case is the impossibility of fixing an already fertilized egg in the uterus.

The solutions to the problem are different. This may be a conservative treatment, but often doctors have to resort to surgery. The woman can get pregnant after removal of the tumor in the uterus, dissection of adhesions, elimination of anatomical defects. If they cannot be eliminated, the patient can always resort to surrogacy.
